BUSINESS PERMIT APPLICATION AND RENEWAL
City Mayor James ‘’Bong’’ Gordon, Jr. is reminding owners, administrators and managers of business establishments in Olongapo City that January of every year is the ‘application, filing and renewal’ month of Business Permits.
‘’Local revenues from business taxes, regulatory fees and other sources provide funds that enable the city to pursue various projects and provide basic social services to the residents of Olongapo,’’ said Mayor Bong Gordon.
According to the Business Permit & Licensing Division Office (BPLDO) headed by Neal Perez, requirements for Business Permit Applications are as follows:
BASIC REQUIREMENTS:
· Duly accomplished application form with 2’’ x 2’’ picture
· Copy of DTI/SEC/CDA Certificate of Registration
· Copy of recent community tax certificate
· Copy of recent official receipt of electric bill payment
CLEARANCES/PERMITS:
· Barangay Clearance (for business)
· Zoning Clearance (City Planning & Development Office)
· Sanitary Permit (City Health Office)
· Fire Safety Inspection Certificate (Bureau of Fire Protection)
· Garbage Clearance ( Environmental Sanitation & Management Office (ESMO)
SPECIAL REQUIEREMENTS:
1. For Owned Properties
Copy of official receipt of Real Property Tax Payment
2. For Leased Properties
Lease contract or Authorization from the owner
3. For Public Market Stallholders
Public Market Clearance
4. For Olongapo City Mall Tenants
Olongapo City Mall Clearance
5. For Computer Shops/ Internet Café/Night Clubs/ Bars/ Billiards
Halls
Affidavit of Undertaking
6. For Corporations/Cooperatives/Partnerships
Authorization to represent the organizations
Meanwhile, renewal of Mayor’s Permit must provide the following documents:
BUSINESS PERMIT ADDITIONAL REQUIREMENTS (for renewal)
• Copy of Precceding Year’s Mayor’s Permit
• Copy of preceding Year’s Official Receipts
• Preceding Year’s gross sales declaration
(supported by Income Tax Return (ITR) and Audited Financial Statements)
Submission of Renewal of Business Permit is up until January 31, 2009 while late payments will automatically have a 25% surcharge and 2% interest per month.
